<p>A 79 years old woman with ventriculoperitoneal shunt (VPS) was diagnosed as acute cholecystitis during hospitalization. Laparoscopic cholecystectomy with the pneumoperitoneum pressure of CO2 set at 12 mmHg was performed without clamping or other manipulations of VPS, considering the risk of breakage. There was no intraoperative and postoperative complication like intracranial hypertension and retrograde infection. Laparoscopic cholecystectomy was thought to be safe against acute cholecystitis with VPS.</p>
