A Case of Trichofolliculoma Arising from the Nasal Vestibule

Description
<p>Trichofolliculoma is a benign tumor of the epidermal appendages with follicular differentiation that commonly occurs on the face. Trichofolliculoma is classified as a skin tumor, and therefore, there are some reports in the dermatology field, but trichofolliculoma in the nasal cavity is rarely reported in otolaryngology. In our case, a 54-year-old male presented with left nasal obstruction. He had noticed a mass in the left nasal vestibule 5 years ago, and the mass had gradually increased in size. A 16-mm elastic hard mass was observed in the 11 o’clock direction in his left nasal vestibule. CT showed a smooth and soft tissue density at the corresponding position, and T1-weighted MRI showed isointensity in a region of high intensity. The tumor and subcutaneous tissue were separated by a low intensity area, suggesting no tumor invasion of the subcutaneous tissue. Surgical resection of the mass was performed under general anesthesia to improve nasal obstruction and obtain a pathological diagnosis. Although the mass was located subcutaneously and adjacent to the major alar cartilage, it was not adhesive to the cartilage and was easily resected. Histopathological findings revealed a central pore with a surface dent and a primary hair follicle structure opening to the epidermis with numerous secondary hair follicles, resulting in diagnosis of trichofolliculoma. No recurrence has been detected for 3 years after the surgery. This case shows that trichofolliculoma can be treated by surgical resection and should be kept in mind as a differential diagnosis in a nasal cavity examination.</p>
