  • 脛骨粗面と膝蓋骨下極の裂離骨折を同時に受傷した成長期サッカー選手の1例

<p>脛骨粗面と膝蓋骨下極の裂離骨折を同時に受傷したまれな1例を経験したので報告する．症例は13歳の男子で，サッカープレー中，シュートする際に軸足側の左膝に疼痛が生じた．前医で4週間の保存加療を施行されたが，膝関節自動伸展障害が残存したため，当科に紹介された．初診時，左膝蓋骨高位を認め，膝関節30°からの自動伸展は不可であった．単純X線およびCT像で，膝蓋骨下極の裂離骨折と脛骨粗面の中枢に小骨片を認め，MR像では，膝蓋腱はすべてこの骨片に付着し，弛緩していた．手術は，吸収性ピン，ならびにsuture bridge法を用いて膝蓋腱付き骨片を脛骨粗面に固定した．膝蓋骨下極の裂離骨折部は安定していたため固定は行なわなかった．術後4.5ヵ月で筋力の回復を確認し，5ヵ月で全体練習に完全復帰した．本骨折は極めてまれで，国内外の報告は，骨端線閉鎖前の症例では7例のみであった．</p>
