Description

A 50-year-old woman was admitted to our hospital with upper abdominal pain. She had been received chemotherapy for multiorgan metastatic relapse of left breast cancer (pT1N0M0 Stage I. ER (+), PgR (+), HER2 (3+) ). She was diagnosed with mild acute cholecystitis by laboratory tests and computed tomography. Although she had a metastatic breast cancer, laparoscopic cholecystectomy was planned because it could be manageable by chemotherapy with maintained performance status. The gallbladder was enlarged and tense, and it had a large white infiltrative nodule on its neck. The cystic duct was completely obstructed by this nodule. In pathological examination, cancer cells with round nuclei infiltrated the all layers of the gallbladder neck. Immunohistochemical examination revealed that the cancer cells expressed ER, and PgR, but not HER2. These results were consistent with breast cancer metastasis. She was uneventfully discharged on day 6, and She is alive 1 year and 10 months from the operation with stable disease of cancer.

Herein, we report a rare case of gallbladder metastasis from breast cancer representing acute cholecystitis.
