<p>An 89-year-old woman was admitted to our hospital with chief complaints of anorexia and malaise. Computed tomography (CT) scan for fever revealed an incarcerated right obturator hernia and fluid collection in the sac, and emergency surgery was performed laparoscopically.</p><p>We found a Richter incarcerated obturator hernia and partially resected a necrotic and perforated ileum. Due to severe inflammation, the hernial sac was not excised and the obturator foramen was not sutured ; therefore, the fallopian tube and mesovarium were sewn onto the hernia porta. On postoperative day 11, the patient complained of fever and right lower extremity edema. CT revealed an abscess from the right obturator foramen to the pelvic cavity ; therefore, incision and drainage of the right thigh were performed under ultrasound guidance. The abscess diminished and lower extremity edema also improved. Here, we report a rare case of femoral abscess characterized by lower extremity edema after the surgery for incarcerated obturator hernia.</p>
