<p>Genital extramammary Paget's disease (EMPD) often develops multicentric lesions. Here, we report three cases of male genital EMPD with multicentric lesions. The cases were between 50 and 70 of age. Multiple lesions were found around the scrotum (7, 5, 5 lesions, each). Dermal invasion was observed in two cases, and sentinel lymph node metastasis was found in one of the two cases. Two cases were considered to be the type that distributes circularly on the periphery of the scrotum (circularly distributed periscrotal type) according to the classification of Kumano et al. The third case was considered to be either a mixture of the deviated peripenile type and the peripheral scrotum type, or an incomplete variant of the circularly distributed periscrotal type. In genital multicentric EMPD of the male, intradermal invasion may easily occur despite the slight clinical findings. Its borders may tend to be unclear, resulting in the existence of occult lesions in clinically normal sites. In multicentric EMPDs, we need to pay enough attention to detect miss micro lymph node metastases and occult lesions.</p>
