A Case of an Incidentally Identified Mesodiverticular Band

Abstract

<p>A two-year-old girl was scheduled to undergo a planned single incision laparoscopic percutaneous extraperitoneal closure (SILPEC) for right external inguinal hernia. Laparoscopic observation revealed a band from the abdominal wall of the navel to the right lower abdomen. After adding a port at the left lateral abdomen, close examination revealed that the band was attached to the ileum mesentery. As we considered the band as a mesodiverticular band, we decided to remove it to prevent small bowel obstruction. The band was coagulated and dissected at the mesenteric attachment and the abdominal wall attachment, and then removed. No Meckel’s diverticulum was identified in the ileum near the band attachment area. Subsequently, LPEC was performed and the surgery was completed. On histopathologic examination, arteries and veins were found in the band. It was diagnosed as a mesodiverticular band. Most mesodiverticular bands are connected to Meckel’s diverticulum. However, in some cases, as in ours, there is a band running from the abdominal wall of the navel to the mesentery of the small intestine, which may not be accompanied by Meckel’s diverticulum. In such cases, we consider that it would be better to remove the mesodiverticular band when we incidentally find it intraoperatively to prevent its onset.</p>
