Abstract

Background. Among the rare extragonadal teratomas, the most common location is the anterior mediastinum. These teratomas can erode adjacent organs through various means; however, perforation into the lung and/or trachea-bronchial tree is rare. Case. This rare case was the first encountered at our respiratory clinic with symptoms of trichoptysis and hemoptysis, and she was eventually treated with surgery consisting of complete tumor removal and right middle lobectomy. Although the preoperative bronchoscopic evaluation and computed tomography (CT) /magnetic resonance imaging/18-fluorodeoxyglucose-positron emission tomography-CT studies predicted a diagnosis of mature teratoma, surgical excision was exceptionally difficult due to obstructive pneumonia and severe adhesion. The patient has not shown any signs of tumor recurrence as of yet. Conclusion. We encountered a unique case in which a preoperative bronchoscopic examination aided in the diagnosis of perforated mediastinal mature teratoma. The tumor was successfully removed.
