Description

A questionnaire for health support and screening of 1st-year students at Kyushu University is presented. The reliability of the questions contained in the questionnaire was evaluated and our findings are here in reported. Each of the questions is answered as either yes or no. The reliability of the questions was examined using Cohen's kappa under test retest conditions. The reliability test revealed that almost all questions regarding both general health conditions and a past history of diseases demonstrated significant and substantial reliability.
