Investigation of the rheumatoid arthritis patients with fracture

<p>Background: Patients with rheumatoid arthritis (RA) are known to be susceptible to bone fractures; however, there are only a few reports on the actual association between RA and bone fractures. Fracture prevention is important in patients with RA to ensure that their physical functions are maintained. </p><p>Subjects: In this study, 53 RA patients with fractures that occurred while the patients were receiving treatment for RA at our hospital were included. The patients ranged in age from 51 to 88 years (mean: 73 years) at the time of their first fracture, and the duration of RA in the patients ranged from 0. 5 to 49 years (mean: 13 years). </p><p>Results: The most common fracture was spinal compression fracture (35%), followed by proximal femur fracture (11%), and fracture of the distal end of the radius (9%), with these 3 fractures accounting for 55% of all the cases with fractures. Of all the patients, 30% suffered from fractures at least twice. No association was observed between the time of occurrence of the first fracture relative to the onset and the duration of RA. At the time of the first fracture, 45% of the patients were receiving drugs for osteoporosis and 70% were on steroid therapy. Among the patients who experienced their first fracture in the relatively early phase (within 5 years) after the onset of RA, the rate of steroid use was high (81%), while that of drugs prescribed for osteoporosis was the lowest (25%). Of all the patients, 77% were eventually discharged to return home after the fracture. </p><p>Conclusion: It is important to pay attention to fracture prevention from the early stage after the onset in patients with RA. </p>
