<p>Benign paroxysmal positional vertigo (BPPV) is classified into canalolithiasis and cupulolithiasis. Each of these shows characteristic nystagmus findings in the positional/positioning nystagmus test; however, the direction, strength, and duration of the nystagmus can change depending on the behavior of the otolith in the semicircular canal. The behavior of otolith on the macula, in the semicircular canals, and on the cupula was shown in the model experiments of BPPV using bullfrogs. By understanding the structure of the inner ear and the properties of otolith and cupula and considering the behavior of the otolith in the semicircular canals or utricle during the positional/positioning nystagmus test, the nystagmus findings could be even more helpful in daily clinical practice.</p>
