A Case of a Huge Duodenal Ulcer with Hemorrhagic Shock Whose Life Could Be Saved

Abstract

<p>A 72-year-old male patient was admitted to the emergency department of our hospital complaining of hematemesis. Upper gastrointestinal endoscopic examination revealed a huge ulcerative lesion in the 1st portion of the duodenum, with bleeding. After the endoscopic procedure, the patient developed cardiopulmonary arrest, but was immediately resuscitated. After return of spontaneous circulation, we performed surgery; we dissected the duodenal margin and performed a distal partial gastrectomy without an ulcer floor and ligation of the hemorrhagic vessels, and mucosal ablation of the residual mucosa. Reconstruction was performed by the Billroth Ⅱ method. The patient was discharged on POD 21 without any adverse events. We report the present case in which partial gastrectomy without ulceration and mucosal ablation was useful in a patient with a huge duodenal ulcer who presented with hemorrhagic shock.</p>
