本研究の目的は,プロセスレコードの分析により,看護婦が患者への対応時に共通して生じやすい問題を検討することにある。対象は「患者指導法の開発」を目的として看護婦から得られた,看護婦の患者に対する好ましい,あるいは問題のある対応のプロセスレコード55事例の中から,特に看護婦が陥りやすい対応を示した4事例を選んだ。分析の結果,事例Aは糖尿病患者が自己管理に対する,ポジティブな行為や行動変容時にも,看護婦の内発的動機づけや共感的理解不足が見られた。事例Bは食道癌術後患者が癌への疑いのため危機的状態にあったが,看護婦の説明的態度が強かった。事例Cは拡張型心筋症で不安状態にある患者に,看護婦は評価的態度で接していた。事例Dは心筋梗塞で治療への予期的不安を持つ患者に対し看護婦は陰性感情を示していた。4事例に共通する対応上の問題は,共感的理解不足による患者の感情表出の抑制である。しかも,事例D以外は看護婦自身は良好な対応と判断していると思われることである。

The purpose of this study is to analyze process records and in doing so, to look at some common problems in how nurses relate to patients. These 4 cases were chosen from among 55 cases specifically because there were such problems in nurses' interactions with patients. Our results are as follows: case A was a diabetic patient who finally began to exhibit good self-control and positive behaviors, but the nurse failed to intrinsically motivate him or develop an empathic relationship with him. Case B was a post-operative esophagus cancer patient who, due to doubts aboute whether or not he had cancer, was in a crisis situation, but the nurse's attitude in explaining his situation was too strong. Case C, a patient suffering from dilated cardiomyopathy, and in an anxious condition, was treated with an evaluative approach by the nurses. In case D, a nurse showed negative feelings toward a patient with acute myocardial-infarction, who was in expectation anxiety. These 4 cases share a common problem, that is, because of a nurse's lack of empathic understanding, the patient was unable to express his feelings. Moreover, except in case D, each nurse seemed satisfied with her handling of the case.
