<p>Heterotopic gastrointestinal cysts are cysts lined with gastric and intestinal mucosa. They are congenital cysts common in the gastrointestinal tract but rare in the oral cavity. We report a case of a 4-year-old girl with the diagnosis of heterotopic gastrointestinal cyst in the floor of mouth. A prenatal diagnosis at another hospital identified a 50 mm cyst in the bottom of her mouth. To prevent airway obstruction, fenestration was performed on the third day after birth. The histopathological diagnosis at the time of fenestration was epidermoid cyst. An excision was planned for after she had grown up, however follow-up was interrupted due to her relocating. At 3 years old, she visited our hospital with a complaint of bleeding from the fenestration, but the visits ended due to hemostasis. She subsequently returned to our hospital with salivary colic-like symptoms. Magnetic resonance imaging revealed a 19 × 17 × 22 mm cystic lesion in the floor of mouth. Excision was performed at the age of 5 and the histopathological diagnosis was heterotopic gastrointestinal cyst. No recurrence has been observed three years after the surgery.</p>
