<p>アナフィラキシーは，急性の全身性アレルギー反応の中で最も重症の臨床的病態である．アナフィラキシーの発生は近年増加しているので，すべての医療従事者がこうした緊急事態に対処できる態勢を整えておくために，その診断と管理に関する知見の普及が継続的に必要である．この改訂版ポジションペーパーを提示する目的は，アナフィラキシーの管理に関する最新の知見に沿った現時点のガイダンスを示す必要があるためである．世界アレルギー機構（WorldAllergy Organization：WAO）アナフィラキシーガイドラインは2011年に発表された．今回のガイダンスでは，そこに示された主要な内容を踏襲し，新たな変更点を取り入れた．エピネフリン（アドレナリン）筋注が今もアナフィラキシーの第一選択治療であることに変わりはないが，その使用は依然として不十分である．アナフィラキシー発症後は患者を専門医に紹介し，考えられる原因の評価と，再発予防および自己管理に関する患者教育が必要である．多くの国でエピネフリン自己注射器の普及が限定的であること，価格的に一部の患者しか手にできないことも，依然として大きな問題である．</p>
