A Case of Small Intestinal Perforation due to Small Bowel Metastasis of Cervical Cancer

Abstract

<p>A 70-year-old woman was brought into the emergency room with a complaint of abdominal pain. An abdominal CT scan showed intra-abdominal free gas and ascites, and her illness was diagnosed as diffuse peritonitis due to perforation of the gastrointestinal tract. The patient had previously been diagnosed with stage IIB squamous cell carcinoma of the cervix 8 months earlier, but surgery and chemotherapy were not indicated because of coexisting scleroderma, pulmonary hypertension, and chronic obstructive pulmonary disease, and only conservative radiation therapy was performed. The surgical examination revealed a tumor perforation in the small intestine about 80 cm anal side from the Treitz ligament, and about 15 cm of the small intestine around the lesion was resected and anastomosed. Gross examination of the resected specimen revealed a white mass at the perforation and thickening of the mucosal surface around the perforation, suggesting a submucosal tumor. Histopathological findings showed squamous cell carcinoma, and a diagnosis of small intestinal metastasis of cervical cancer was made. A small bowel metastasis of the cervical cancer is very rare, and we found only 5 reports in the Japanese literature. Yet, it should be kept in mind as the differential diagnosis of acute abdomen in patients with a history of cervical cancer.</p>
