Abstract
<p>In 2017, our hospital assigned a pharmacist in charge of regional collaboration, and since 2020, we have been working as a team of multiple pharmacists. We investigated the effects of team intervention on medication adherence and the number of regular oral medications. A total of 274 patients were selected as subjects. These were patients who had shared information with insurance providers. Regarding medication adherence, the percentage of patients evaluated as having no problems after hospital discharge was significantly increased in the intervention group (p < 0.001). Regarding the number of regular oral medications, there was a tendency for reduced medication (9.0 drugs to 7.3 drugs) in the intervention group, with a significant decrease when compared with that in the non-intervention group (6.5 drugs to 6.5 drugs) (p < 0.01). Team intervention may be expected to improve medication adherence and proper drug use after discharge from the hospital.</p>
