We report two cases of vertebral osteomyelitis with ileus-like symptoms. One was a 68-yearold Caucasian man with a history of cholecystectomy, who presented with vomiting for a duration of 5 days.Initially diagnosed with postoperative adhesive ileus and bacteremia, intravenous (IV) cefmetazole was started. He left the hospital against medical advice but was later readmitted and finally diagnosed with vertebral osteomyelitis. The second patient was a 69-year-old Japanese man who presented with a history of vomiting for 4 days and lumbago for 10 days.Although initially suspected as a case of ileus, investigations led to a final diagnosis of vertebral osteomyelitis. Both cases showed no signs of colonic obstruction. There are few case reports of vertebral osteomyelitis with ileus-like symptoms. Here, we report two such cases and discuss the mechanisms of vertebral osteomyelitis, which causes ileuslike symptoms, with a literature review.
