<p>The Mayo-Portland Adaptability Inventory-4 (MPAI-4) is designed for the comprehensive evaluation of individuals with acquired brain injury (ABI), encompassing physical, cognitive, emotional, behavioral, and socio-environmental challenges. MPAI-4 could be administered independently of the cooperation of the patient. The Japanese MPAI-4 version has recently been published, we thus hereby report its usefulness and prospects based on our clinical experience.</p><p>In Case 1, the MPAI-4 systematically elucidated the challenges and strengths of a patient with ABI resistant to treatment and assistance, providing insights for the interdisciplinary team to deliver tailored support. In Case 2, the MPAI-4 functioned as a shared framework for interpreting patient characteristics across diverse contexts.</p><p>Case study-based clinical observations indicate that the Japanese MPAI-4 version comprehensively evaluates the various symptoms and living situations of patients with ABI in their home environments without overlooking any aspects. Moreover, this inventory facilitates information sharing and unified support strategy establishment across multiple facilities and professional domains. Nonetheless, the validity of the Japanese adaptation is currently under evaluation, holding promise as a standard tool for fostering intra- and interfacility collaborations in individuals with ABI as a prospective avenue.</p>
