Description

Objective: As Japan becomes a super-aging society, male hypogonadism, particularly late-onset hypogonadism (LOH syndrome), is an area in which treatment and research are expected to develop in the near future. Therefore, our health examination center uses the Aging Males' Symptoms (AMS) score, which is used to screen for LOH syndrome in the examination items. In this study, we investigated the usefulness of the AMS score by examining the frequency of LOH syndrome in participants undergoing the Ningen Dock examination and its relationship with clinical variables, such as blood free testosterone (FT) level.

Methods: The participants were 601 men (mean age; 52 ± 10 years) who underwent Ningen Dock examination from June 2020 to May 2021. Blood FT concentration was measured using the RIA/solid-phase method, oxidative stress levels were measured using previously reported d-ROM and BAP tests, and NT-proBNP was measured using an electrochemiluminescence immunoassay. Other clinical variables were determined using automated analytical methods. Blood pressure was determined using an automatic upper arm cuff method, pulse wave velocity (PWV) was measured using PWV test device, bone mineral density (BMD) was measured using dual-energy X-ray absorptiometry, and visceral fat area (VFA) was determined using computed tomography diagnostic device at the umbilicus level.

Results: The prevalence of LOH syndrome was 3.3% in AMS score ≥50 and 14.0% from 37–49. The AMS scores were significantly positively correlated with age (p < 0.0001), VFA (p = 0.0005), systolic blood pressure (0.0015), PWV (0.0045), NT-proBNP (0,0141), and AST (0.0266). FT (p < 0.0001), BAP (0.0045), and BMD (0.0344) were significantly negatively correlated with AMS score. Multiple regression analysis of AMS score showed that age (0.00028), triglycerides (0.0282), FT (0.02759), and albumin (0.03456) were independent explanatory variables.

Conclusion: The prevalence of LOH syndrome was 17.3%. The onset of LOH syndrome is associated with FT level, antioxidant capacity, and age. These results suggest that testosterone replacement may suppress the onset and progression of LOH syndrome and increase antioxidant capacity.
