<p>Background. Intravascular large B-cell lymphoma (IVLBCL) is a rare extranodal non-Hodgkin lymphoma characterized by malignant cells that invade and proliferate within systemic microvessels. This condition often presents with hypoxemia and is challenging to diagnose and treat. Case. A 73-year-old man presented to a nearby hospital with dyspnea on exertion and was subsequently referred to our hospital. Upon admission, he exhibited mild interstitial opacities on chest high-resolution computed tomography (HRCT), type 1 respiratory failure, markedly elevated serum lactate dehydrogenase (LD) levels, and decreased platelet count. Despite the initiation of steroid therapy, his respiratory failure progressed. On day 6, a random skin biopsy was performed, and on day 7, a transbronchial lung biopsy (TBLB) was performed. Based on the pathological findings of both specimens, the patient was definitively diagnosed with IVLBCL and achieved remission after chemotherapy. Conclusion. We report a case of IVLBCL complicated by acute respiratory failure that was diagnosed by a TBLB and a random skin biopsy. A definite diagnosis of IVLBCL requires biopsies from multiple sites. When pulmonary lesions are present, performing TBLB may be warranted alongside non-invasive biopsy techniques, provided that safety is ensured.</p>
