Opalski syndrome (OS) is a rare variant of Wallenberg syndrome characterized by ipsilateral hemiplegia. OS is caused by post-decussating pyramidal tract damage. The associated lesion is the lateral medulla oblongata or the area from the lateral medulla to the lateral aspect of the upper cervical cord. Some studies have described the lesions and clinical findings of OS. However, to our knowledge, there are no reports on the progress and prognosis of the clinical findings of OS. Few studies exist on the rehabilitation of OS patients. Consequently, understanding OS is challenging for rehabilitation personnel. Rehabilitation personnel should be able to differentiate ipsilateral ataxia and hemiplegia from OS, so that appropriate assessment, goal setting, and rehabilitation treatment can be provided. Herein, we have summarized 43 studies on OS, including the literature related to OS, and described the characteristics, progress, and prognosis of the clinical findings of OS.
