Rapidly developing preauricular malignant melanoma with dedifferentiation : A case report

<p>We report an 81-year-old man who developed malignant melanoma with dedifferentiation.</p><p>A purple-red tumor appeared in front of the patient’s left ear. It developed rapidly over several weeks. Hematoxylin and eosin staining of the biopsy specimens showed an alveolar-like structure in the dermis with proliferation of atypical cells of varying sizes. Immunostaining revealed positivity for Melan A, HMB45, and S100.</p><p>After 2 weeks, he was referred to our hospital and the tumor was removed. The size of the tumor was 50×40×30 mm. The histological examination showed proliferation of epithelial-like tumor cells, with weakly stained eosinophilic cytoplasm through hematoxylin and eosin staining. Immunostaining revealed negative results for the aforementioned melanocyte markers.</p><p>These findings were inconsistent with those of the biopsy.</p><p>Dedifferentiation of malignant melanoma is extremely rare and a relatively new concept. Therefore, it may be misdiagnosed as another sarcoma. Awareness of this concept is important for accurate diagnosis.[Skin Cancer (Japan) 2019 ; 34 : 144-148]</p>
