Description
A 67-year-old woman with lower abdominal pain presented to our hospital. Her medical history included laparotomy for ovarian cystoma. Contrast-enhanced abdominal CT showed that the greater omentum was thickened and highly enhanced with angiectasis. She was diagnosed with omental panniculitis and underwent antibiotic treatment and steroid therapy. She was referred to our department, and diagnostic surgery was performed because the therapies were in effective. Laparotomy showed a large tumor originating from the greater omentum. The tumor was diagnosed pathologically as malignant during surgery. There were numerous white nodules on the mesentery of the small intestine. These findings indicated peritoneal dissemination from omental tumor, and only cytoreductive surgery was performed. The final pathological examination confirmed a diagnosis of malignant mesothelioma derived from the greater omentum. She was uneventfully discharged from our hospital. She died 73 days after operation due to rapid growth of the malignant mesothelioma. Malignant mesothelioma arising from the peritoneum including the greater omentum is relatively unusual, and proper diagnosis is difficult without tissue biopsy. A case of primary malignant mesothelioma of the greater omentum is reported along with a review of the relevant literature.
