A Case of Necrobiotic Xanthogranuloma as Initially Diagnosed to Be Cutaneous Sarcoidosis

Abstract

An 80-year-old Japanese man consulted our hospital for dark reddish papules and plaques on the extremities. Based on the histopathological finding of naked granulomas in the dermis, we diagnosed the eruption as cutaneous sarcoidosis. During the 18-month application of topical corticosteroids, the eruption deteriorated to ulceration. We performed a secondary skin biopsy. Many Touton-type giant cells with necrobiosis were noted in the dermis, and we diagnosed him with necrobiotic xanthogranuloma (NXG). In addition, we found monoclonal gammopathy. His skin lesion was improved by oral prednisolone. We speculate the pathogenesis of NXG to be due to the involvement of macrophages, similar with cutaneous sarcoidosis.Skin Research, 17: 70-74, 2018
