A 66-year-old woman was admitted to our hospital with complaints of bloody stools. Endoscopic examination revealed a type 1 tumor in the lower rectum. We performed laparoscopic abdominoperineal resection and classified the tumor as cStage I (T2N0M0). Macroscopic findings of the resected specimen showed a type 1 tumor with no abnormal findings in the surrounding mucosa and skin. Pathological examination revealed Paget cell infiltration of the epidermis surrounding the tumor. Immunostaining was performed, and a diagnosis of anal canal adenocarcinoma with Pagetoid spread (PS) was made. The maximum diameter of the PS was 102 mm, and pathological staging was classified as pStage I (pT2pN0M0). An accurate diagnosis of lesion spread is important for anal canal cancer with PS, but in this case, preoperative evaluation was difficult because of a lack of macroscopic abnormal findings. We report this rare case because we consider it noteworthy.
