<p>Questionnaire survey for adult cochlear implantation was carried out among 109 facilities which enrolled as cochlear implant center in the Oto-rhino-Laryngological Society of Japan, Inc. A cochlear implantation was hoped for a patient less than 90dB in 85% of facilities. A cochlear implantation was performed to a patient less than 90dB in 48% of facilities. 82% of facilities thought that the revision of indication criteria of 1998 for cochlear implant was necessary. As for the lightest mean hearing threshold level to cochlear implantation, higher 91dB was 20. 7%, 81-90dB was 51. 7%, 71-80dB was 14. 9%. Doctors thought that the phone definition was important to decision of indication to cochlear implant in 93% of facilities. 67% of facilities have carried out bilateral cochlear implantations. Based on this questionnaire findings, it was thought that revision of indication criteria for adult cochlear implant was necessary.</p>
