Abstract

<p>A 79-year-old man, who had been treated with topical steroids under the diagnosis of psoriasis for several decades, was admitted to our department because of a worsening of psoriatic lesions on his extremities and trunk. We started treating him with etretinate at 20 mg/day and topical steroids of the ‘very strong’ class together with vitamin D3. In addition, whole body narrow-band ultraviolet B phototherapy (one session per week at a dose of 300 mJ/cm2 ) was conducted, leading to a favorable response. One and a half years later, he complained of appetite loss and a 15 kg reduction in body weight within 2 months. Computed tomographic scanning showed numerous manifestations of lymphadenopathy in the neck, axilla, mediastinum, and groin. Hypercalcemia was noted but anti-HTLV-1 antibody was not detected. Histopathological findings of atypical lymphocytes with some granulomatous lesions in the left inguinal lymph node led to the diagnosis of peripheral T cell lymphoma (PTCL). Eight cycles of CHOP (cyclophosphamide, doxorubicin, vincristine, prednisolone) therapy resulted in a complete remission of lymphoma. Psoriasis has been known to be a potential risk factor for PTCL, presumably due to the malignant transformation of psoriasis-associated T cells. Therefore, attention should be paid to the association of psoriasis with PTCL.</p>
