<p>A 77-year-old woman had right leg cellulitis for the first time in October 2015. Although the initial treatment was successful, the cellulitis recurred seven times during the next year. She had poorly controlled diabetes and tinea unguium of the toenails. She previously had an operation for a fracture at the neck of the right femur. She did not have chronic venous insufficiency or lymphedema that could cause recurrent cellulitis and there was no abnormality on gross inspection. Cellulitis of the leg is a common bacterial infection of the skin and underlying tissue. Especially, recurrent cellulitis is known to be accompanied by chronic venous insufficiency, lymphedema, and tinea pedis. There have been several reports that prophylactic low-dose penicillin can prevent the recurrence of cellulitis. She started treatment with prophylactic low-dose penicillin in February 2017. The cellulitis recurred once, 3 months later, but it did not recur again during the next year. Therefore, prophylactic low-dose penicillin seemed to be effective in decreasing the relapse rate of recurrent cellulitis. However, the protective effect diminished progressively once drug therapy was stopped. The optimal long-term dosage and the risk of developing bacterial resistance are unclear. We should consider whether to use prophylactic low-dose penicillin for patients with recurrent cellulitis.</p>
