A Case of Early Gallbladder Carcinoma with Acute Cholecystitis Caused by Tumor Impaction in the Neck

Abstract

A 64-year-old woman was admitted to our hospital with epigastralgia and nausea. Physical examination revealed mild tenderness in the right upper abdomen. CT revealed edematous thickening of the gallbladder wall, and we diagnosed the patient as having acute cholecystitis. Because a mass was found in the gallbladder neck, we considered that the acute cholecystitis was caused by impaction of the mass. Although we started the patient on antibiotic treatment, the right upper abdominal pain and inflammatory findings only worsened. Emergency laparotomy was performed on the third day of hospitalization. We incised the cystic duct near the junction of the common bile duct and resected the gallbladder. In the resected specimen, a soft pedunculated tumor measuring 3 cm in diameter was found in the gallbladder neck. No additional resection or lymph node dissection was performed. Histopathological examination revealed early gallbladder carcinoma and no cancer progression in the cystic duct stump. Gallbladder carcinoma often manifests with non-specific subjective symptoms, and early cancer cases are often found incidentally in specimens obtained after cholecystectomy performed for cholelithiasis. This report describes a rare case of early gallbladder carcinoma with acute cholecystitis caused by tumor impaction in the neck, with a review of the literature.
