<p>Background. Sarcoidosis with atelectasis is rare in Japan. Case. A 68-year-old woman presented with dyspnea on exertion. Chest CT showed granular shadow in the bilateral upper lobes, bronchial stenosis, and right middle lobe atelectasis. Positron emission tomography-CT revealed a high fluorine-18 deoxyglucose (FDG) accumulation in the right middle lobe, so we performed a bronchoscopic examination. Bronchoscopic findings showed an obstruction at the orifice of the right middle lobe bronchus and we performed a biopsy. The tissue diagnosis was compatible with sarcoidosis. She is under careful observation as an outpatient. Conclusion. Sarcoidosis is rare as a cause of atelectasis, but should be considered.</p>
