Evaluation of secular trends of recurrence of proximal femoral fracture from the data of clinical pathway with regional alliance in Hata district

Abstract

<p>OBJECTIVES: Secular trends of recurrent proximal femoral fracture (PFF) were evaluated from the data of clinical pathway with regional alliance (CPRA).</p><p>METHODS: The CPRA data for PFF from July 2011 to March 2017 were harvested for this study. The number of recurrences, time span between initial fracture and recurrence, and fundamental data of recurrent cases during the initial fracture were statistically analyzed and compared to non-recurrent cases.</p><p>RESULTS: Among 1118 cases, 45 cases (4 men and 41 women; average age, 85.71 years) showed recurrence including 6 cases of ipsilateral fracture. The average interval period between initial fracture and recurrence was 13.31 months, of which recurrence in 30 cases occurred within one year of the initial fracture. The ratio of withdrawal was significantly higher than that of the non-recurrent cases, mainly due to death and serious co-morbidity.</p><p>DISCUSSION: Most recurrent fractures occurred within one year of the initial fracture. Despite research of the risk factors causing recurrence, no factors demonstrated a significant difference between early recurrent cases and the others. Although it is important to prevent early recurrent fractures, no effective strategy could be suggested.</p><p>CONCLUSIONS: The recurrence of PFF is severe and its prognosis is remarkably poor, and majority cases may occur within one year of the initial fracture.</p>
