Description

According to the classical theory, the pupillary light reflex results from a simple subcortical reflex arc. In other words, postgeniculate lesions in the visual pathway are of no immediate relevance to the pupillary light reflex. However, many previous studies reported that patients with isolated occipital lobe lesions and homonymous visual field defects showed pupillary hemihypokinesia, i.e. reduced or absent pupil light responses to perimetric stimuli in the blind part of the visual field. Published findings have provided compelling evidence of the role of cortical processing of the pupillary light reflex under certain stimulus conditions. This article verifies the old hypotheses of pupillary hemihypokinesia based on the new findings.

Since the discovery of melanopsin-containing retinal ganglion cells, more studies have been performed, and new findings have been reported. This article reviews these new findings and describes the general standards of pupillography.
