Description

A 25-year-old Japanese female presented with a skin lesion on her left nipple. The lesion had been present for three months and was increasing in size. A physical examination revealed a brown hyperkeratotic verrucous nodule on her left nipple. A histological examination of a biopsy specimen showed hyperkeratosis, papillo-matosis, and acanthosis. There were several koilocytes in the upper epidermis. Positive staining for human papillomavirus (HPV) particles by immunohistochemical analysis was seen in several cell nuclei at the upper malpighian layer. Mucous-type HPV DNA was detected in the lesion using the polymerase chain reaction (PCR), and it was identified as HPV type 6 by direct sequencing of the PCR product. We treated the patient by cryotherapy with liquid nitrogen, and the lesion disappeared. Condyloma acuminatum of the nipple is very rare. Interestingly, most reported clinical cases exhibit a seborrheic keratosis-like appearance. Dermatologists should consider condyloma acuminatum in cases in which seborrheic keratosis-like lesions are shown on the nipple.
