Abstract

<p>特に既往症のない 79 歳，男性。四肢に環状の紅斑が出現し，その後，胸痛，多発関節炎，有痛性皮下結節，発熱，リンパ節腫脹，上強膜炎が次々に生じ，最終的に，初発の皮膚症状の 4 カ月後に生じた耳介腫脹により，再発性多発軟骨炎の診断に至った。気道や心病変は合併しておらず全身状態は良好であるが，ステロイド内服と免疫抑制剤の併用では，いまだ病勢はコントロールできていない。<br>再発性多発軟骨炎は，軟骨組織を主体に多彩な全身性の自己免疫性の臨床症状を呈し，寛解再燃を繰り返す。半数以下の症例に皮疹を伴うが，皮疹自体も多様で特異的なものはない。自験例の様に，軟骨炎や鼻軟骨炎などの典型的な症状がない病期での診断は非常に困難である。</p>
