Abstract

<p>54歳，女性．肝硬変，乳糜胸水貯留でA病院より紹介．悪性疾患の併存はなく，胸部手術歴や外傷歴もないこと，胸水TG（triglyceride）値，胸水リポ蛋白分析より，非外傷性乳糜瘻と診断．絶食・中心静脈栄養で胸水は減少したが，食事再開後，増加．MRI（magnetic resonance imaging）で腹部胸管での乳糜漏を疑い，リピオドールリンパ管造影を施行．以降，胸水は減少，オクトレオチド併用でほぼ消失した．リピオドールリンパ管造影とオクトレオチドは非外傷性乳糜胸水の有効な治療と考える．</p>
