<p>扁平上皮化生を伴う甲状腺乳頭癌は時に扁平上皮癌や未分化癌との鑑別を要する。病理医と臨床医が協力することで適切に未分化癌と鑑別することができた症例を経験した。患者症例は66歳女性。乳頭癌（cT1bN0M0 StageⅠ）の術前診断にて甲状腺左葉切除，中心領域リンパ節郭清術を施行した。病理組織学的所見では一部に乳頭状構造が不明瞭で，甲状腺未分化癌を否定できない所見だった。同部位を免疫組織化学法を用いて精査したところ，扁平上皮マーカーのp63，p40は陽性だがKi-67が低値であり悪性度は低く，扁平上皮化生を伴う甲状腺乳頭癌と診断された。</p>
