抄録

Heart failure is caused by various cardiovascular diseases including ischemic heart disease, hypertension, cardiomyopathy, valvular heart disease, and congenital heart disease. The number of heart failure patients is increasing according to aging population, the increase of life-style related diseases such as hypertension and diabetes mellitus, and the improvement of acute management for acute coronary syndrome. Particularly the number of aged patients, who require hospitalization due to worsening heart failure, are increasing, which is now regarded as the major society as well as medical issues. / The onset and progression of heart failure are complexly influenced not only by its causes and comorbidities but also the social factors. The observational study utilizing registry is essential to establish the effective management guidelines. Studies utilizing registry have been reported from the United States and Europe, which, however, cannot be applied to Japanese patients, whose race, age, and healthcare system are different from western patients. Therefore, the study based on our own registry is definitely needed. / The study utilizing registry could contribute to establish more effective and efficient management by elucidating not only the patientsʼ characteristics in real world clinical practice, but also by identifying the predictors for prognosis and assessing the efficacy of treatment. It is also needed to investigate the long-term changes and international comparison of heart failure. It is now expected for us to organize the registry data system for heart failure and build the database platform of registry which includes medical records and DPC data. This database platform shall be utilized widely for the academic research, the development and evaluation of drugs and devices, and the planning of medical policies and proper allocation of medical expenses.
