A Case of Omentum Liposarcoma Undiagnosed Preoperatively and Resected Laparoscopically

Abstract
<p>A 37-year-old woman presented with an intrapelvic tumor detected by an abdominal ultrasonic study at a medical checkup in November 2018. An abdominal CT scan revealed a 120-mm unenhanced intraabdominal tumor positioned on the cephalad site of the urinary bladder, with a homogeneous inside, low density and well demarcated border. An abdominal MRI scan also showed the inside of the tumor to be T1 low and T2 high. Although findings suggestive of malignancy were lacked, the tumor was comparatively large and a possibility of malignancy could not be ruled out completely. Keeping a possible conversion to an open surgery in mind, we decided to perform laparoscopic resection for the purpose of diagnostic treatment in February 2019. It was found that the tumor was present in the greater omentum, no dissemination nor adhesion was seen, and the tumor could be resected laparoscopically. The inside of the tumor was solid, and the tumor was diagnosed as well differentiated liposarcoma. The patient is doing well as of 1 year and 6 months after the operation. No cases of laparoscopically resected omental sarcoma have previously been reported in Japan. Laparoscopic resection for diagnosis may be permitted for abdominal tumors undiagnosed preoperatively if it is safely carried out, however, we must bear a possibility of malignant disease such as liposarcoma in mind and must pay careful attentions not to cause rupture or dissemination of the tumors.</p>
