<p>A 37-year-old man was admitted to our hospital with a day’s history of abdominal pain. A plain X-ray of the abdomen showed fluid levels and bowel obstruction was suspected. Abdominal CT showed distended and poorly enhancing loops of small bowel and a band, associated with caliber change of the small bowel at the site. We suspected strangulated bowel obstruction due to internal herniation and performed an emergency operation. At laparotomy, Meckel’s diverticulum was identified, located in the ileum 40 cm proximal to the end of the ileocecal valve, and a band was seen, originating from the top of Meckel’s diverticulum and adhering to the mesentery of the terminal ileum, forming a potential hernial orifice. Since the ischemic changes of the incarcerated ileum recovered after the band was released, only diverticulectomy was performed. The histopathological diagnosis of the resected cord, containing vessel components, was mesodiverticular vascular band(MVB). Intestinal obstruction caused by a MVB is rare, but this possibility should be considered in the differential diagnosis of intestinal obstruction, especially in patients without a previous history of laparotomy.</p>
