<p> Placental surface cysts are cystic lesions that develop on the fetal surface of the placenta and have little effect on the course of pregnancy; however, an association with fetal growth restriction and maternal collagen diseases has been reported. We report a case of fetal growth restriction due to placental cysts during pregnancy management for dermatomyositis. Ultrasonography at 28 weeks of gestation revealed many cystic lesions on the fetal surface of the placenta, with the largest being 6.0 cm in diameter. Fetal growth restriction of -2.4 SD was observed at 30 weeks of gestation and emergency cesarean section was performed under a diagnosis of fetal dysfunction. A female baby weighing 1,043 g was delivered(Apgar score 6/9), but there was no abnormality in the postnatal course. Fibrin deposition and X-cells were suggested to be related to the development of placental cysts, and placental pathological findings in this case demonstrated fibrin deposition around the cysts and X-cell proliferation.</p>
