A Non-invasive Continuous Cardiac Output Measurement Method Utilizing ECG and SpO_2 Pulse Wave

We developed a non-invasivecontinuous cardiac output measurement (CCO) method utilizing ECG and SpO_2 pulse wave based on pulse contour analysis combined with pulse wave transit time (PWTT). PWTT is calculated from the interval between the ECG R wave and peripheral SpO_2 pulse wave arrival. We found a correlation between PWTT and stroke volume (SV) in an animal study. The estimated CCO (esCO) was derived as follows : esCO=Κ×(α×PWTT+β)×HR α is a constant. Κ and βare derived by only one calibration at the start of measurement. Whereas the CO derived from Pulse Pressure based on the conventional pulse contour analysis was found to overestimate the CO after the administration of phenylephrine, the esCO was unaffected and accurate. We compared esCO with the thermodilution CCO using a pulmonaly artery catheter (CCOpa) in an ICU. The bias between them was -0.06±0.82L/min. The bias between esCO and bolusCO (BCO) were comparable to that between CCFOpa and BCO. The bias was -0.63±1.01L/min between esCO and BCO, and -0.64±0.84L/min between CCOpa and BCO. This result suggests that esCO has potential as an alternative non-invasive cardiac ooutput.
