Abstract
<p> Recently, the number of elderly primiparas has been increasing in Japan due to the tendency towards marrying later and the progress of reproductive technology. We investigated a retrospective study to clarify the risk of primiparas over 40 years old. 1,690 primiparas who delivered at our institution between 2017 to 2020 were classified into 3 groups according to their maternal age at delivery. Cesarean section rates increased with increasing maternal age, and there was no difference in their reasons. The number of vaginal delivery due to labor without intervention decreased with increasing maternal age, and the prognostic factors were being under 35 years old, embryo transfer pregnancy, and preeclampsia. The amount of bleeding during labor increased with age, and the prognostic factors were pregnancy by embryo transfer and using uterotonics in vaginal delivery, and placenta previa and previous uterine surgery in cesarean section cases. There was no difference in neonatal outcomes between the groups. It is important to manage labor and bleeding appropriately, considering individual risk factors including maternal age and their background.</p>
