Abstract

肺移植の適応となる拘束性肺疾患の代表は，IIPsである。脳死肺移植の場合は，片肺移植のよい適応となるが，残存肺の気胸や感染症の合併症が問題となる。脳死ドナーの少ない日本においては，特に進行の速いIPFなどに対しては，生体肺移植がしばしば唯一の救命法となる。扁平胸郭を伴うpleuroparenchymal fibroelastosis（PPFE）や，膠原病に合併した間質性肺炎に対しても肺移植が適応となることがある。
