Abstract

<p>アメリカ合衆国パラメディックは，病院前救護で多彩な医療行為を実践する。このパラメディックを養成する1施設の調査を行った。全米標準カリキュラムに基づいた教育プログラムと学生64名の臨床経験を調査した。課程期間44週間で学生は516時間の講義とシミュレーション手法を含む実習，病院実習234時間（以下，中央値），救急車同乗実習329時間，救急車でのインターンシップ80時間を経験した。この間，学生は成人患者136人，高齢者82人，小児43人の傷病者・患者，それに伴う病態を多数経験した。指導および監督下で，学生は実際の傷病者・患者に対して経口気管挿管5回，静脈路確保86回，薬剤投与106回を含む侵襲的処置を実施した。救急車同乗実習では学生が主体でチーム指揮，傷病者対応を行う実践的実習が実施されていた。米国の標準的パラメディック養成教育は長時間の病院と救急車での臨床的経験を提供し，侵襲的処置を自ら判断，実施するon-the-job教育が行われている。</p>
