<p>【Purpose】To report a case of acute syphilitic posterior chorioretinitis that was erroneously followed up for a long time as acute zonal occult outer retinopathy.</p><p>【Case】The patient in this case was a 39-year-old man with a chief complaint of photopsia in the left eye in X (month) 2019, which had been followed up as acute zonal occult outer retinopathy for 13 months. Subsequently, he was referred to our department for a definitive diagnosis. The findings at the first visit included decreased macular reflex and redness of the optic nerve head in the fundus of the left eye, irregularity of the ellipsoid zone revealed by OCT, and fluorescence leakage and multiple fluorescence accumulation in the optic nerve head detected by a fluorescence contrast examination. The result of a blood test confirmed the diagnosis of syphilitic uveitis.</p><p>【Conclusion】Ocular syphilis has no characteristic clinical findings and that often delays diagnosis and the initiation of appropriate treatment. When a definitive diagnosis is difficult to make, syphilis should be considered as one of the differential diagnoses.</p>
