National Survey on the Long-Term Course of Pediatric Tracheostomy Patients

Abstract

<p>Ensuring long-term airway management and respiratory control is vital for pediatric patients who have undergone tracheostomies. Due to the various complications that may arise during the procedure, several innovations are necessary for the tracheostomy technique and postoperative management. Additionally, no established protocol exists for decannulation tracheostomies in children, which is largely dependent on the experience of each institution. With the development of perinatal and pediatric care, the number of pediatric patients undergoing tracheostomies is expected to increase. Thus, data and protocols to guide the management of these patients need to be established. In this study, we conducted a national survey aimed at achieving safe airway management and tracheal closure in pediatric patients undergoing tracheostomies. We received responses from 57 of 202 eligible facilities. A total of 29 centers had performed pediatric tracheostomies in the past decade. In the last year alone, a total of 196 pediatric tracheostomies were performed. The technique and preoperative course of each institution's 20 most recent cases were examined. Furthermore, data on long-term complications and tracheostomy decannulation in the past decade were collected. The incidence of complications was higher in children than in adults, and the rate of tracheostomy decannulation was approximately 10%. Thus, it is desirable to deepen understanding of long-term management and tracheostomy decannulation through future studies.</p>
