Abstract

<p>  The position of ports in laparoscopic surgery restricts the “direction” of the forceps. Thus, the position of the ports is usually determined by taking into consideration “by who” and “how” each port will be used. In nephrectomy, four ports are set at retroperitoneal, trans-peritoneal, left, and right, and those are basically used as the camera port, surgeon’s right-hand port, left-hand port, and assistant’s port. The first camera port is always set in the open laparotomy. Further, the “right-hand port” is usually set at 12 mm, so that renal hilum can be performed with the dominant hand using a vascular device. The 5 mm ports are used where a 10 mm device would be unnecessary. The port incision should be made as much as possible as a transverse incision in line with the assumed skin incision line, in case a laparotomy is required. Sample collection should be performed by extending the camera port (transverse incision) and using muscle split as much as possible, without considering connecting port incisions.</p>
