Abstract

<p>症例 1：77 歳，男性。左前頭部の血管肉腫に対し，外科的切除後パクリタキセルによる維持化学療法を行うも，肺，肝臓，リンパ節転移とともに血小板減少をきたした。抗血小板抗体陰性，血小板結合性免疫グロブリンG（PA-IgG）陽性，DIC スコア 5 点であった。初診から 10 カ 月後に死亡した。症例 2：93 歳，男性。左頭頂部の血管肉腫に対し，放射線治療，パクリタキセルによる維持化学療法を行うも，肺，後頚部リンパ節転移，血小板減少をきたした。PA-IgG 陽性，DIC スコア 5 点であった。初診から 20 カ 月後に死亡した。2 例とも多発遠隔転移が出現し，腫瘍部での血小板消費により，Kasabach-Merritt 症候群と同様の機序で血小板減少をきたしたと考えた。当院で 2012～2021 年に経験した進行期に血小板減少を伴った 3 症例では，血小板減少を伴わなかった 25 症例と比較すると多臓器転移の割合が高かった。</p>
