<p> Actinomycosis of the jaw is relatively rare, and few cases presenting in the maxilla have been reported. It has recently become difficult to diagnose actinomycosis bacteriologically because of the use of antibiotics. We report a case of a palatal fistula caused by maxillary actinomycosis. The patient was a 68-year-old man who presented with a palatal fistula. Computed tomography showed bone destruction of the hard palate and sequestrum in the nasal fossa. Histopathological findings revealed actinomyces infection. Antibiotics were administered, but the fistula did not resolve. Primary surgery was performed to remove the sequestrum. After 12 months, a second surgery was performed to close the fistula with a mucoperiosteal flap on the oral side and a hinge flap on the nasal side. No recurrence was observed during the 12-month follow-up period.</p>
